On or about August 3, 2016, at the facility located at 2131 Deckner Ave. Green Bay, WI 54302, the flexible cord from the portable air conditioning unit used to cool the "Hospital Services" room was not installed in a manner that was consistent with National Electric Code.  The cover of the panel board was not completely installed and the cord was run into the panel board between the cabinet and the bottom of the partially installed cover.
