29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: On or about August 4, 2016, at the worksite located at 4910 Stagecoach Road in Little Rock, Arkansas, employees on a flat roof were exposed to unprotected falls of greater than 6 feet (a) when engaged in roofing activities such as but not limited to roof removal, repair and PVC roof membrane installation, and (b) when entering and exiting the Roura trash hopper. These conditions exposed employees to the hazard of falls from elevations.
