29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: Employees voluntarily wearing filtering facepiece disposable respirators were not provided with a copy of Appendix D. This condition exposed employees to health hazards. Advanced Bath and Kitchen, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(k)(6), which was contained in OSHA inspection number 1115823, citation number 2, item number 1 and was affirmed as a final order on March 8, 2016 with respect to a workplace located at 1010 Jessie Road in Little Rock, Arkansas.
