29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: 123 Kelly Ave, Fitchburg, MA As of August 5, 2016, the employer had not instituted an energy control program with energy control procedures, training, and periodic inspection. The lack of the program exposes employees potentially hazardous energy while servicing the Wysong Hydraulic press and other machinery.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): 123 Kelly Ave, Fitchburg, MA On August 5, 2016 and at times prior, the Wysong Hydraulic Press was not properly guarded allowing operator access to the point of operation. a. On front of machine, light curtains had 2-7/8 inches of blanking installed that allowed operator access underneath the curtains b. On the sides of machine, there was a 12 inch gap with no guarding between back of light curtains and the machine.
