Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) 601 Willow Grove Street, Hackettstown, NJ 07804: Employees who were engaged in roofing work on a large flat roof more than 6 feet above the ground were not protected by an adequate fall protection system. This hazard was observed on 8/3/16.
