29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy.    a). Bag Department - On or about and times prior to August 3, 2016, adequate procedures were not developed to protect employees from the slitter blades on the forming section of Bag Machines # 8, 9, 10, and 11 when they were re-energized and the jog function was used to ensure the paper was fed through the machine properly, thus exposing employees to laceration and amputation injuries.    AJM Packaging Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i), which was contained in OSHA inspection number 1030948, citation number 1, item number 1 and was affirmed as a final order on August 11, 2015, with respect to a workplace located at 3401 Southeast Boulevard, Vineland, NJ 08360.    OR IN THE ALTERNATIVE    29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s).    a). AJM Packaging Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1) which was contained in OSHA inspection number 1030948, citation number 1, item number 3 and was affirmed as a final order on August 11, 2015, with respect to a workplace located at 3401 Southeast Boulevard, Vineland, NJ 08360.
