OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crush-by and struck-by hazards: At the marina, at least (4) maintenance employees operated a Galion Model 150A Series S/N 7194 crane to conduct activities such as repairing and maintaining of boats and lifted personal. The employer did not ensure the safety items found during an inspection were repaired prior to requiring employees to operate the crane: 1. Crane was not equipped with an ant-two block safety device. 2. Crane hook was not equipped with a safety latch. 3. Load Hook Dead Ending Rope length incorrect. 4. Load Hook Dead Ending Rope was unraveled.
