29 CFR 1910.134(d)(3)(iii)(B)(2):  The employer did not implement a change schedule for respirators not equipped with an ESLI that would ensure cartridges were changed before the end of their service life:       a)  On or about August 5, 2016, and at times prior, and at times prior, the employer did not protect employees that used half-face air purifying respirators while spray-applying coatings containing isocyanates and other hazardous chemicals by developing and implementing a cartridge change schedule to prevent breakthrough of the organic vapor cartridges used with the respirators. 29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials, a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:    a)  On or about August 5 2016, and at times prior, employees were exposed to fire and explosion hazards in that the employer did not store the oxygen cylinders 20 feet away from acetylene cylinders and other combustible materials, such as paper in the trashcan.
