29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded to prevent the operator from having any part of his body in the danger zone during the operating cycle. This violation occurred on or about July 12, 2016, when an employee operating a Clam Shell, Friends Model 28 X 40 was exposed to crushing and/or amputation hazards.

29 CFR 1910.212(a)(3)(iii): Special handtools for placing and removing material did not permit easy handling of material without the operator placing a hand in the danger zone. Such tools will not be used be in lieu of other guarding required by this section, but can only be used to supplement protection provided. This violation occurred on or about July 12, 2016, when an employee was operating a Clam Shell, Friends Model 28 X 40 was exposed to crushing and/or amputation hazards.
