29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented: a) 250 Grandview Avenue, Edison, NJ 08837. Mr. Juan Lopez, the employer allowed his employees to conduct rip and tear roofing operation as well as install new roofing materials with no fall protection system in place exposing the employees to fall hazards. Occurred on or about 08/09/2016.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identified them as defective, or tagged with Do Not Use or similar language, and were not withdrawn from service until repaired: a) 250 Grandview Avenue, Edison, NJ 08837. Mr. Juan Lopez, the employer allowed his employees to access the roof of a one story home using an aluminum extension ladder that was extensively damaged and pieced together when it was not marked faulty or defective or tagged "Do Not Use" and removed from service exposing the employee to fall hazards in the event of failure. Occurred on or about 08/09/2016.
