29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) North side of house - On or about July 21, 2016 employees applying wood shingles to steep roof 27 feet high were not protected from falling by using a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) North side of house - On or about July 21, 2016 employees installing wood roof shingles to steep roof used an aluminum extension ladder that did not extend 3 feet past roofs edge to prevent employees from falling while using the ladder.
