OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught in-between and amputation hazards: a) Halliburton Energy Services at the Spaur Well Pad #10Y-241, Milliken, CO, 80543: On or about July 1st, 2016, employees removing and replacing suction valves on a Q10-QSK50 high pressure pump were exposed to caught in-between and amputation hazards after disassembling and placing their hands inside the plunger bore cylinders while the unit remained running. While doing so, one of the plungers unexpectedly stroked forward, crushing the left hand and fingers of an employee. Among other feasible and acceptable abatement methods to correct this hazard is to ensure that all employees and supervisors follow the employer's written Lockout Tagout program (WM-GL-HAL-PE-425) for high pressure pump maintenance.
