29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)    At the 2 Roseberry Circle, Pt. Wentworth jobsite: On July 21, 2016, and at times prior thereto, employees were exposed to fall hazards of up to 28 ft. 6 in, in that roofers without fall protection were engaged in shingle-application activities on a 7:12-slope roof.     b)    At the 2 Roseberry Circle, Pt. Wentworth jobsite: On July 21, 2016, and at times prior thereto, an employee was exposed to fall hazards of about 8 ft., in that a roofer without fall protection was engaged in connecting an air compressor on a 4:12-slope porch roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a)    At the 2 Roseberry Circle, Pt. Wentworth jobsite: On July 21, 2016, and at times prior thereto, employees were exposed to fall hazards of up to 17 ft. 4 in, in that roofers used a 32-ft. portable ladder with side rails not extend at least three (3) feet above the upper landing, and not secured at the top, to access and egress a steep roof.
