29 CFR 1926.451(b)(1): Where the employer made a demonstration that a space wider than 1 inch was necessary between adjacent plank units and the platform and uprights, the platform was not planked or decked as fully as possible. (a) North side of house - On or about July 29, 2016 employees were exposed to a 42 foot fall hazard while framing a house from scaffolding that was not fully planked.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: (a) North side of house - On or about July 29, 2016 employees working on scaffolding 42 feet high were not protected from falling by the use of guard railing or a fall protection system.
