29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about August 3, 2016:  a) Employees utilizing an aerial lift to preform painting operations were exposed to fall hazards of approximately 25 feet when the employer failed to ensure that employees were using fall restraint systems.       In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:  On or about August 3, 2016:  a) An employee working form an aerial lift was sitting on the railing exposing him to a fall hazard of approximately 25 feet, while preforming painting operations.       In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  On or about August 3, 2016  a) Employees utilizing an aerial lift to preform painting operations were exposed to fall hazards of approximately 25 feet when the employer failed to ensure that employees were properly trained to recognize and reduce or eliminate fall hazards.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
