29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    (a) Inside Garage - On or about August 5, 2016 employee working from self made wood scaffolding attaching second floor rafters 12 feet high was not protected from falling with the use of fall protection or railings.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.     (a) Inside Garage east wall - On or about August 5, 2016 an employee was using an aluminum folding ladder that did not reach the top of the 13 foot side wall to gain access to and from the upper level.

29 CFR 1910.178(l): At this jobsite, the employer had not assured that the forklift operators were competent to operate the JLG N77592 forklift, which was being used to transport and place wood framing material on the upper floors, as demonstrated by the successful completion of  the training and evaluation specified in this paragraph (l).    (a) West side of house - On or about August 5, 2016 an employee was operating the JLG N77592 rough terrain fork lift to supply material to the workers on the second floor with out having received any training on the save operations of fork lifts.

29 CFR 1926.405(g)(2)(iii): The repair splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used:    (a) West side of building - On or about August 5, 2016 The ROLAIR air compressor electrical cord has been spliced at the plug end using standard black electrical tape to cover the wires that does not meet the insulation of the outer sheathing.
