29 CFR 1926.451(c)(2)(v):       Fork-lifts were used to support scaffold platforms where the entire platform was not attached to the fork.    (a) On or about August 9 , 2016 at  worksite 9617 N Elm Ct, Kansas City, Missouri.-employees were exposed to a fall hazard when performing carpentry actives from a man made scaffold platform made up of plywood sitting a top of forks, at an elevation of approximately 25-27 feet above the surface absent of any fall protection epuipment .        Abatement certification not required for this item.

29 CFR 1926.501(b)(1):      Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      (a) On or about August 9 , 2016 at  worksite 9617 N Elm Ct , Missouri.-employees were exposed to a fall hazard when performing carpentry actives at an elevation of approximately  10-22 feet from the surface below, without an approved form of fall protection, to include, guardrail systems, safety net systems, or personal fall arrest systems.          Abatement Note: Abatement certification for this violation is not required.
