29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  On or about 8/9/2016 in the production area at 269 Industrial Park Drive, Monticello, GA:  Employers energy control procedures for their machines and equipment were not clear, specific, and did not address steps to safely de-energize all of the energy sources such as but not limited to electrical, pneumatic, steam, heat, and mechanical. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about 8/9/2016 at the foam cutting machines in the production area at 269 Industrial Park Drive, Monticello, GA:  Employees operating the foam cutting machines with the heated metal wires used to make the vertical and horizontal cuts to the foam material were exposed to temperature up to 138 degrees Fahrenheit.  The employer failed to guard the heated wire while the employees were removing the access foam or working near the connected areas of the machine. 29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  On or about 8/9/2016 at the blue horizontal and vertical foam cutting machine labeled A10 in the production area at 269 Industrial Park Drive, Monticello, GA:  Employees operating the cutter machine were exposed to approximately 120 volts on the unguarded metal sections of the machine.  An employee was hospitalized from the electrical shock he sustained from the unguarded metal section on 8/2/2016.
