29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) Front left side of porch: On August 9, 2016, employees were exposed to a fall of 10 feet, porch edge to ground, while installing brace boards on the porch.  Employees were not protected from falling by a guardrail system, safety net or another suitable fall protection system.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) Front Roof: On or about August 9, 2016 and times prior, an employee was exposed to a fall of 13feet 6 inches, from eave to ground while working on the roof installing sheeting.  The employee was not protected from falling by a guardrail system, safety net system or personal fall arrest system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) Front of home: On or about August 9, 2016, an employee was exposed to a fall of up to 13 feet while using a 20 foot Werner extension ladder to access the roof to install sheeting.  The ladder was not extended 3 feet above the roof edge.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a) Front Porch: On or about August 9, 2016, an employee used a 6 foot Werner step ladder, in the closed position and leaned up against the porch wall, while installing porch bracing.  The base of the ladder was not fully opened with the spreaders in the locked position.
