On or about August 9, 2016 one employee was observed installing metal gutter downspouts on a new one story commercial structure while standing on a wood pallet installed on a Genie GTH-644 telescopic Forklift and was exposed to a fall in excess of 12 feet to the ground below because he was not protected by any means of personal fall protection harness, guardrails or safety net.
