On 8/9/2016, employees from Continental Steel Erectors, Inc. were repairing a roof at the Remodeling Room located at 1901 Scherer Parkway, Saint Charles, MO. The foreman and four employees were observed working on the roof, approximately twenty-two feet above the ground, without the use of guardrail systems, safety net systems, or personal fall arrest systems.
