29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  a) On or about August 3, 2016 at 28013 State Hwy 181, building 3300, Daphne, Al: The employer failed to ensure the work area around building 2900 was maintained free of debris and wood with protruding nails which exposed employees to the hazards associated with slips, trips, and falls.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a) On or about August 3, 2016 and times prior at 28013 State Hwy 181, building 3300, Daphne, Al: The employer failed to ensure an employee taking measurements and  installing sheathing on the roof of a residential apartment building adjusted his rope grab as he moved allowing additional slack to accumulate allowing a fall of more than 6 feet.

29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.  a) On or about January 28, 2016 at 2101 Berry Hill Rd., Montgomery, Al., building 3300: The employer did not ensure employees using personal fall arrest were properly anchored in that their rope was wrapped around a support beam and the snap hook was connected back to the rope while taking measurements and installing sheathing which exposed them to falls of 30 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about August 3, 2016 and times prior at 28013 State Hwy 181, building 3300, Daphne, Al: The employer failed to ensure employees involved in roof sheathing activities had fall protection in place exposing them to a fall hazard of 9.3 feet to the ground below.  Elvis Euceda was previously cited for a violation of this occupational safety and health standard or its equivalent standard for residential fall protection, which was contained in OSHA inspection number 1130873, citation number 01, item number 01 and was affirmed as a final order on April 18, 2016, with respect to a workplace located at Pelican Landing Apartments Bldg. 3 & 5, Mobile, Al.
