29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: A). On or about 08/09/16, job site - In the front of 221-23 Hartland Ave. Queens Village, NY 11427 Employee working inside a completed section of the trench approximately 20' x 8' x 14' deep was hit on the forehead by a broken wood plank divider. The wood plank divider was pushed by the bucket of the Komatsu PC228 excavator digging into the active section of the trench. ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
