29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of a powered industrial truck were performed by the employer without the manufacturer's prior written approval: a.) Cedar Point Amusement Park, located at 1 Cedar Point Dr. Sandusky, OH; On or about August 10, 2016, the employer did not ensure modifications such as a Tulsa winch installed on the back of the Pettibone Industrial Tractor was performed with manufacturer's approval. Employees are required to use the Pettibone Chugger to help move rollercoaster cars out of valleys, and are exposed to hazards associated with PIV and winch use.

Cedar Point Amusement Park, located at 1 Cedar Point Drive Sandusky, OH; On or about August 10, 2016, the employer did not ensure each operator was evaluated on the powered industrial vehicle equipment, including but not limited to the Pettibone Chugger. Employees were exposed to struck by hazards while using the chugger to pull train cars out of valleys of roller coasters.
