29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment: On or about August 9, 2016 and times prior thereto, at the worksite located at or near 70 Elk Way in Mountain Home, Arkansas, the employer did not provide adequate eye protection to employees working with liquid chemicals in the maintenance shop. This condition exposed employees to splash hazards.

Employees are required to work with gasoline, diesel, oil, and other hazardous substances. This exposed employees to chemical hazards.

29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. On or about August 9, 2016 and times prior thereto, at the worksite located at or near 70 Elk Way in Mountain Home, Arkansas, the employer did not record the following a workplace injury on the OSHA Form 300 or equivalent for the calendar year 2016: An employee got hit in the head while removing tree limbs from the golf course. The injury resulted in the employee losing consciousness and lost work days.
