29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    Southeast, building 2, creasing-scoring machine: on or about August 10, 2016, operators were hand-feeding corrugated sheets into inadequately guarded point-of-operation where provided horizontal physical barrier guards was not secured (swinging), and also not adjusted down (lowered) to the thickness of material being processed to prevent operator's access in accordance with OSHA Table O-10.     Used two-hand controls were not configured for concurrent cycle actuation and were not safely separated by at least 21 inches in single linear dimension to prevent by-passing. Operator's finger(s) and hand(s) were exposed to machine caught-in hazards likely to result in serious injury.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
