The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.        a)    On or about 8/8/2016, An employee was injured while reaching into the point of operation of the CD02, Cold Header #2 built by CENTENNIAL TECHNOLOGIES, INC.  The Cold Header was not guarded as the wiring had been malfunctioning causing the two hand controls to not protect the operator.
