29 CFR 1910.147(f)(2)(i): Whenever outside servicing personnel were to be engaged in activities covered by the scope and application of this standard, the on-site employer and the outside employer did not inform each other of their respective lockout or tagout procedures.   a) ASARCO Hayden Smelter, Converter Baghouse area: Outside servicing employees of Ace Pipe Cleaning were performing maintenance on the energized rotary Baghouse hopper while the onsite employer ASARCO LLC and Ace Pipe Cleaning did not inform each other of their respective lockout or tagout procedures. Employees were exposed to amputation hazards from inadvertent contact with rotating paddles.

OSH Act of 1970 Section 5(a)(l): The Employer did not furnish employment and a place of employment which were free  from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards while cleaning hoppers:    a) Motors and appliances: At the converter baghouse at rotary valve module 6 east, disconnecting means were not readily apparent and were not legibly marked. On or around August 4, 2016, an employee's hand was stuck in the rotary valve and it took approximately 45 minutes to de-energize the machine and release the employee.

29 CFR 1910.1025(h)(2)(i): Floors and other surfaces where lead accumulates were cleaned by the use of compressed air.     a) ASARCO Hayden Smelter, Converter Baghouse area: Compressed air was used to remove lead accumulations in the Converter Baghouse Hoppers and surrounding area.  Employees were exposed to airborne lead accumulates.

Carylon Corporation dba Ace Pipe Cleaning Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1910.132(d)(2), which was contained in OSHA inspection number 398262, citation number 2, item number 1 and was affirmed as a final order on August 1, 2012, with respect to a workplace located at ASARCO Hayden Smelter, 64 Asarco Ave., Hayden, Arizona 85235.

The employer did not develop, document and enforce the use of specific procedures in order to control potentially hazardous energy.  Employees were exposed to amputation hazards from contact with moving paddles.      Carylon Corporation dba Ace Pipe Cleaning Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 398262, citation number 2, item number 3 and was affirmed as a final order on August 1, 2012, with respect to a workplace located at ASARCO Hayden Smelter, 64 Asarco Ave., Hayden, Arizona 85235.
