Edinboro Industries, Inc., 4200 Rt. 6N East, Edinboro, Pa, 16412, CNC Lathe shop area, on or about August 11th, 2016, and times prior thereto:  The flexible electrical cord used to provide electrical power to the Band Saw Welder attachment to the Walker-Turner vertical band saw (Model # 3331; Serial # 0000157) was frayed on the outer jacket and contained a damaged strain cap.
