29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   a) On or about 08/11/2016 at the job site, an employee was exposed to a 20 foot fall hazard, in that, fall protection equipment was not used while installing plywood panels on the side of the building from a JLG 800AJ, serial # 682168 aerial lift.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)  On or about 08/11/2016 at the Hibiscus apartment complex building A, employees were exposed to a 20 ft. fall hazard, in that, fall protection was not used. The lanyards were not connected to the harnesses and/or lifeline. Employees were installing plywood panels on a 5/12 pitched roof.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.   a) On or about 08/11/2016 at the job site, no certification of training was provided that verified employees had received training to recognize fall hazards and procedures to follow or minimize fall hazards. The employer did not establish written records of training for the employees.
