Baldwin Ranger District, Rec Warehouse, fire aisle leading to the fire exit was blocked by materials such as but not limited to boxes of materials, buckets and Personal Protective Equipment (PPE) there by exposing employees to a fire hazard.

29 CFR 1910.178(l)(4)(iii) as required by 1960.8 (b): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:     Mio Ranger District, the employer failed to re-evaluate Powered Industrial Truck Operator performance. Employee's certification was issued on June 9, 2010 with no expiration date. The operator used the Clarklift forklift S/N 040B-960-893 on August 10, 2016 to offload a pallet of material from a truck thereby exposing to employees to a struck by hazard.

29 CFR 1910.303(b)(2) as required by 1960.8 (b):  Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    The employer did not ensure that relocatable power tap (RPT) were not daisy chained to other RPTs or connected to extension cords.    a. In the Dispatch office, three different relocatable power taps were daisy chained to other relocatable power taps used to power electronic equipment such as but not limited to computers, radio charging stations and telephones.  b. In the Biology Resource office, an extension cord was daisy chained to a relocatable power tap used to power electronic equipment such as but not limited to computers and telephones.  c. In the Science Resource office, an extension cord was daisy chained to a relocatable power tap used to power electronic equipment such as but not limited to computers and telephones.
