29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available, and the methods and means necessary for energy isolation:    A)    Daily Services, LLC DBA iforce, LLC, employing workers at 20 Baird Parkway, Mansfield, OH  44903:  On or about May 9, 2016, and at times prior thereto, the employer failed to perform authorized employee training, or ensure that authorized employee training was performed for temporary workers prior to allowing them to engaged in servicing and maintenance activities on equipment.  Employees were not trained to isolate hazardous energy sources prior to engaging in servicing activities such as, but not limited to changing welding tips on the OTC DR-4000 Weld Robots, which were located inside of robotic welding cell #10058.  As a result of this failure, employees were exposed to struck-by and crushing hazards.
