29 CFR 1926.1060(a)(1): The employer did not provide a training program for each employee using ladders as necessary, enabling each employee to recognize hazards related to ladders and the procedures to be followed to minimize these hazards:  (a) On or about August 5, 2016, and at times prior, Esquivel Roofing Limited did not provide effective ladder training to ensure the recognition of ladder hazards and the procedures to be followed to minimize these hazards. Employees were exposed to fall hazards of up to 13 feet while using ladders to access roofs from a single family residential home. 29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  (a) On or about August 5, 2016, the employer did not protect employees from fall hazards or other serious injury due to defective ladder not being withdrawn from service. Structural components at the bottom of the base section of the extension ladder were contorted causing the feet to face inward. The Defective ladder was being used to access the roof for re-roofing operations on a single family home.
