On or before August 11, 2016, where employees used an aerial lift to access windows between the third and fourth floors, the employees were not protected from falls, thereby being exposed to 30 foot fall hazards.

29 CFR 1910.132(f)(1): The employer shall provide training to each employee who is required by this section to use PPE. Each such employee shall be trained to know at least the following: a. On or before August 11, 2016, where employees used an aerial lift to access windows between the third and fourth floors, the employees were not trained on the use of personal protective equipment for fall protection in aerial lifts, thereby being exposed to 30 foot fall hazards.
