29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:     a)  On August 11, 2016, employees were not protected against accidental contact with energized overhead power lines during roofing work.  The overhead power lines consisted of secondary lines having a voltage rating of 120/240 volts.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a)  On August 11, 2016, employees engaged in roofing activity were not protected from fall hazards while working at an elevation of approximately 13 feet above the ground.  No abatement certification or documentation is required for this item.
