29 CFR 1910.147(a)(2)(ii): Alternative measures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by the exception of 1910.147(a)(2)(ii) for routine, repetitive, and integral processes such as tool change, change over, and cleaning. ESTABLISHMENT: The employer did not develop, document and utilize alternative measures for the control of the potentially hazardous energy when employees were required to perform set up of lead rolls and/or clean rollers on machines, such as (but not limited to) Team (Nord) machine and Slitter #3 machine.
