29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about August 11, 2016 the employer failed to develop, document and utilize adequate energy control procedures when loading the nail feeder (servicing) the Viking machine. Employees were exposed to struck-by hazards and caught in between hazards while performing this routine task. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1910.19(c)

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about August 11, 2016 the employer did not ensure that employees loading the nail feeder were required to perform the following sequence of actions prior to servicing the Viking Machines: 1) Machine or equipment shutdown 2) Machine or equipment isolation 3) Lockout or tagout device application 4) Verification of isolation. Employees were exposed to struck-by hazards and caught in between hazards while performing this routine task. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1910.19(c)

Employees were exposed to caught-in between and struck-by hazards. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
