29 CFR 1926.652(b): The slopes and configurations of sloping and benching systems were not in accordance with the requirement options of 29 CFR 1926.652(b)(1), (b)(2), (b)(3), or (b)(4):    On or about August 3, 2016, and at times prior thereto, the employer had not designed the sloping and benching for a trench greater than 5 feet in depth for employees installing pipe splicers on an 8-inch PVC sewer pipe, including but not limited to allowable slopes, configurations for sloping and benching, designed by a registered professional engineer.  This condition exposed employees to cave-in hazards.
