29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   a) 4729 Onondaga Blvd, Syracuse, NY, On or about 8/10/16: The employer failed to provide adequate safety monitoring system as a means of  fall protection while employees were installing roofing on a flat roof that was less than 50 feet in width or depth and had a fall distance of 13 feet.  The Pyramid Roofing and Sheetmetal Co, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10)), which was contained in OSHA inspection number 517678 , citation number__1______, item number___2a____ and was affirmed as a final order on 3/28/16 with respect to a workplace located at 109 Wyoming St, Syracuse, NY.  The Pyramid Roofing and Sheetmetal Co, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10)), which was contained in OSHA inspection number 915012  , citation number__1______, item number___1____ and was affirmed as a final order on 1/6/2014 with respect to a workplace located at Liverpool Plaza, Old Liverpool Rd, Liverpool, NY.
