29 CFR 1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance: On or about August 9, 2016, employees were exposed to struck-by hazards when the lockout/tagout procedures said that the supervisor was to apply the locks to the equipment rather than the employee who was working on the equipment. Employees performed work such as, but not limited to, removing drops, cleaning around the laser equipment, changing the dust collector barrels, lubricating the robot welders, and changing tips on the spot and robot welders. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished: On or about August 9, 2016, employees were exposed to struck-by hazards when the lockout procedures said that the supervisor was to verify that lockout was effective instead of the employee performing work on the equipment. 29 CFR 1910.1200(g)(8): The employer did not ensure that Safety Data Sheets were readily accessible during each work shift to employees when they were in their work area(s): a. On or about August 9, 2016, the safety data sheets for the chemicals located in the corner near the exit door and used in the clean/prep equipment at 3644 Burnette Road in Suwanee, GA were located in the office in a white binder (COR ADD 27 290-038 Detergent Additive, CORAL ECO-TREAT NPLF 130-013 Multi Metal Conversion Coating). They were not in the MSDS binder in the production area or available to the employees. b. On or about August 9, 2016, there were no safety data sheets for the powder paints used at 3644 Burnette Road in Suwanee, GA. (POWDURA TGIC polyester powder coating (various colors) and POWDURA super durable TGIC free polyester powder coating (various colors) c. On or about August 9, 2016, the safety data sheets for propane and acetylene were located in the office at the Sweetwater Park Drive site and had never been placed in the MSDS binder or made available to the employees. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
