29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Ohio Carbon Industries, DBA Buckeye Molded Plastics located at 443 Oberlin Road in Elyria, Ohio; On or about August 8, 2016 the employer failed to ensure employees were protected from amputation hazards in that the pinch point created as the top punch passes through the die plate of the Baldwin Press, was not adequately guarded to prevent employees from accessing the hazard area at a time the press could operate.
