29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:   Production area, on or about August 16, 2016 and at other times prior to, employees utilizing half face respirator where the employer failed to do a hazard assessment on personal protective equipment to determine if hazards are present in the workplace to include chemical hazards which may cause injury through absorption, inhalation, or physical contact.
