On or about August 4, 2016 and at times prior to, between generator 2 and 3, an electrical cable tray was in the hoist way of an overhead crane and was not protected from physical damage which exposes employees to electrical hazards.

29 CFR 1926.62(d)(1)(iii): The employer did not collect personal samples representative of a full shift, including at least one sample for each job classification in each work area, either for each shift or for the shift with the highest exposure level:    a.  On or about August 4, 2016 and at times prior to, at the power plant, the employer did not require a contractor to collect personal samples to determine lead exposure during tasks which include, but are not limited to manual scraping, chemical stripping, needle gunning, and/or grinding of metal parts of turbines and generators where lead-containing coatings are present.     b.  On or about August 4, 2016 and at times prior to, at the power plant, the employer did not require a contractor to collect personal samples to determine lead exposure during tasks which include, but are not limited to torch cutting or torch heating of metal parts of turbines and generators where lead-containing coatings are present.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a.  On or about August 4, 2016, by turbine 3 at the downstream wall, the raceway that held energized electrical cables lacked a cover and the energized electrical cables were not guarded to prevent employee contact which exposes employees to electrical hazards.      b.  On or about August 4, 2016, by turbine 1 at the downstream wall, the raceway that held energized electrical cables lacked a cover and the energized electrical cables were not guarded to prevent employee contact which exposes employees to electrical hazards.

29 CFR 1926.1127(d)(1)(i): The employer did not establish the applicability of this standard by determining whether cadmium was present in the work place and whether there was the possibility that employee exposure will be at or above the action level prior to performance of any construction work:    a.  On or about August 4, 2016, and at times prior to, power plant, the employer did not require a contractor to establish the applicability of the standard, did not determine that cadmium was present in paint-primer on turbine parts including but not limited to servo motors, and did not determine whether there was the possibility that contractor employee exposures would be at or above the cadmium action level during manual scraping, chemical stripping, grinding, needle gunning, and torch heating or cutting painted metal turbine parts.
