29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a)Iron Leaf, LLC, Edison, NJ site - facility: The employer did not establish an Energy Control program that defines machine specific energy control procedures, training and periodic inspections for employees performing part change out, maintenance and/or servicing machines to prevent accidental energizing of the machines. Machinery including but not limited to the Komatsu PC250LC - 6L Hydraulic Excavator, Hitachi EX200LC Hydraulic excavator, and the CAT 246C Skid Steer Loader did not have written procedures for de-energizing, and isolating energy sources. Condition noted on or about 8/16/2016. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19. (LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(IDENTIFY MACHINERY OR EQUIPMENT)
