Thomas Tree Service, Inc., shop at 7190 Canal Road in Fairview, PA. On or about August 1, 2016, employees performed servicing and maintenance work including, but not limited to, clearing jams from the Cord King Compact wood processor. Specific lockout procedures were not developed for the Cord King Compact wood processor. The generic lockout/ tagout procedures for the wood processor did not contain specific procedures and techniques to prevent the unexpected energization of moving parts including, but not limited to, the saw blade and/or preventing the saw from falling while in the up position.
