29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) On or about 8/15/16, at the Elk Terminal Lofts Construction Project, 230 Scott St., Buffalo, NY:) An HDY Single Work Light was located in the corridor outside the bathroom in unit 37 A, and was used to provide light to the area in the bathroom where ceramic tile was being installed. The Work Light was connected to 120 volt-ac power via a series of extension cords, the first of which was plugged into a receptacle in a temporary power distribution center. The last cord in the series was a short 3-Way GFCI extension cord that lacked a ground prong on its plug. NO ABATEMENT CERTIFICATION REQUIRED
