29 CFR 1910.147(e)(1): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not inspect the work area to ensure that nonessential items had been removed and that machine or equipment components were operationally intact: a) Maintenance Shop, on or about August 5, 2016, and at times prior too, the employer did not ensure the cover plate was on the Boston Mathews belt puller machine gear box before restarting the equipment, thus exposing employees to amputation hazards.
