29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut: a) On or about 8/8/2016, SAW STOP table saw was being operated without the hood guard in place when an employee cut his hand on the saw blade while cutting Medium Density Fiberboard (MDF). The SAW STOP saw was a 10" Industrial Cabinet Saw, Model No. ICS53480, Serial No. I121700802.

29 CFR 1910.151(b): In the absence of an infirmary, clinic, or hospital in near proximity to the workplace which is used for the treatment of all injured employees, a person or persons shall be adequately trained to render first aid. Adequate first aid supplies shall be readily available. a) On or about 8/8/2016, company did not have a person trained in first aid to respond to an employee injury or medical help available in near proximity to the site.
