Sandusky St. Upper Sandusky, OH: The employer did not ensure an employee working from an aerial lift was adequately protected from falls. The employee working from the lift was not clipped off to the basket while the platform was elevated. The employee was exposed to falls of approximately 25 feet to the ground below.

Employees painting within arm's length of the 220V energized electrical service line were exposed to electrical shock/electrocution.
