29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:    (a) Nick Oney on or about August 16, 2016, at the address of 136 N. Sandusky St. Upper Sandusky, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identify and avoiding potential hazards, such as fall and electrical shock/electrocution during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a)  Nick Oney on or about August 16, 2016, at the address of 136 N. Sandusky St. Upper Sandusky, OH: The employer did not ensure an employee working from an aerial lift was adequately protected from falls.  The employee working from the lift was not clipped off to the basket while the platform was elevated. The employee was exposed to falls of approximately 25 feet to the ground below.

Sandusky St. Upper Sandusky, OH: The employer did not ensure employees were effectively protected from live conductors of an energized electrical service line by means of deenergization, insulation or proximity guarding during commercial painting operations. Employees painting within arm's length of the 220V energized electrical service line were exposed to electrical shock/electrocution.

29 CFR 1926.1053(b)(12): Ladders used where the employee or the ladder could contact exposed energized electrical equipment did not have nonconductive siderails:  (a)  Nick Oney on or about August 16, 2016, at the address of 136 N. Sandusky St. Upper Sandusky, OH: The employer did not ensure a non-conductive ladder was used when employees were operating in the immediate vicinity of a 220V electrical service line. Employees working from and near the aluminum extension ladder were exposed to electrocution while performing painting operations within arm's length of the energized line.
