29 CFR 1910.1048(l)(3)(i): The employer did not administer a medical disease questionnaire, such as in Appendix D of this standard, designed to elicit information on work history, smoking history, any evidence of eye, nose, or throat irritation; chronic airway problems or hyper-reactive airway disease: allergic skin conditions or dermatitis; and upper or lower respiratory problems:    a) Formaldehyde Tanker Truck Loading Area - On or about October 5, 2016,  Employees  loading tanker trailers were exposed to formaldehyde at a 15 minute time weighted average of 2.84 parts formaldehyde per one million parts of air (ppm).  This exposure is 1.42 times the Short Term Exposure Limit (STEL) of 2.0 ppm.  This exposure was observed occurring over a 7 minute sampling period on 10/5/16.  A zero exposure was assumed for the 8 minutes not sampled.  The employer did not provide/administer a medical disease questionnaire for employees over exposed to formaldehyde which met the requirements of this section.
